For example, other rights such as publicity, privacy, or moral rights may limit how you use the material.</p> Fracturas de cadera en adultos mayores: estrategias quirúrgicas y resultados funcionales https://sergeykrayev.net/index.php/ojs/article/view/386 <p><strong>Introducción:</strong> Las fracturas de cadera en adultos mayores constituyen un problema relevante de salud pública por su asociación con discapacidad, pérdida de independencia y mortalidad. <strong>Objetivo:</strong> Sintetizar críticamente la evidencia sobre las principales estrategias quirúrgicas y los resultados funcionales en adultos mayores con fractura de cadera. <strong>Metodología:</strong> Se realizó una revisión sistemática con síntesis cualitativa, guiada por PRISMA 2020. Se consultaron PubMed y SciELO y se incluyeron estudios originales y revisiones con población de 65 años o más que evaluaron tratamiento quirúrgico, funcionalidad, complicaciones o mortalidad. Doce estudios fueron incluidos. <strong>Resultados:</strong> En pacientes previamente independientes y clínicamente seleccionados, la artroplastia total tendió a ofrecer una ventaja funcional frente a la hemiartroplastia, aunque con mayor complejidad quirúrgica y posible incremento de complicaciones específicas. La osteosíntesis mostró resultados favorables en fracturas extracapsulares estables. La cirugía sin demoras evitables, la valoración geriátrica integral y la rehabilitación precoz se asociaron con mejores desenlaces. Conclusiones: La elección del procedimiento debe individualizarse según el patrón de fractura, la función previa, la fragilidad, la expectativa de vida y el riesgo perioperatorio.
